Who Needs One:
- Incoming 7th Graders
- Incoming Freshman
- Incoming Juniors
- Anyone that wants to participate in athletics that did not turn in a physical for the 2021-2022 School year.
What to Bring:
-Pre-participation Physical Form
-Concussion Acknowledgement Form
-Sudden Cardiac Arrest Awareness Form
- Parent/Student Steroid Agreement Form
- $20 Cash
- All forms must be COMPLETED and SIGNED by parent/guardian before receiving physical
Forms will be given out to students by coaches/athletic trainer the week prior to physicals. All UIL medical forms are also available at https://www.uiltexas.org/athletics/forms
