CISD Technology Check Out Agreement
Payments for the CISD Technology Optional Self-Insurance may be made online on our district website, https://www.caldwellschools.net/apps/form/optionaltechnologyinsurance. There is a 4% convenience charge for paying online with a credit card. If you would prefer to pay by check or money order, please mail or deliver the check to 203 N Gray Street. Please make the check payable to Caldwell ISD. Indicate in the MEMO it is for Technology Device/Equipment, and be sure to include a note stating the child(ren)'s first and last name(s) and grade(s) for which you are purchasing the insurance. You may also pay at your child's campus.
*It is highly discouraged to pay with cash; however, if you have no other option, please make sure you ask for a receipt and keep your receipt for your records.

The deadline to purchase the insurance is October 15, 2021. Please be aware if you do NOT purchase the insurance by this deadline, you WILL be responsible for the cost of the repair or replacement of all the device/equipment.
