Athletic Physicals

Incoming 7th, 9th, and 11th grade students involved in UIL (athletics, band, cheer, honeybees) MUST have an updated physical on file.

Please see the opportunity for Sports Physicals at CISD. Paperwork MUST be completed in order to get physical on site. Please complete the Google Form AND Registration (below) if interested in utilizing this opportunity.

Thursday, May 2 at CJH from 9am-3pm
Tuesday, May 7 at CHS from 9am-3pm
